| Oil giant BP plc was aware of problems with maintenance, spending and infrastructure at its Texas City refinery well before an explosion and fire killed 15 contractor employees and injured another 170 workers in 2005, the U.S. Chemical Safety Board says in releasing preliminary findings of its ongoing probe of the accident.
